Runner-runner refers to a draw that required two cards to hit, not just one. Classics are runner-runner flushes and straights that cause the losing player to go on uber-tilt and proceed to play badly and lose a lot of money.
They also often result in a stream of juicy abuse being directed at the lucky player who hit their runner-runner.

Someone like spoon holds JJ on a J74 flop. Villain holds TT. After much posturing and betting, both players get all-in on the flop. Turn comes T, river comes T and runner-runner quads beat top-full-house.
